Aadhaar Form

It is mandatory in India to have an Aadhaar Card to prove your identity and residential status. Many Banks, Insurance Companies, and other institutions ask for Aadhaar Card as an identity and address proof but you will have to first apply for an Aadhaar Card. There is no way to get an Aadhaar Card online because you will have to visit the nearest Aadhaar Seva Kendra and fill out the Aadhaar Form to get the Aadhaar Card. Contents of the Aadhaar Application Form

There are 11 empty fields in the Aadhaar Form (Link Aadhar PDF) and it is not mandatory to fill all the fields. You can go through the contents of the Aadhaar Card Form and see the information that has to be filled in every field.

Acknowledgment Number

When you book an appointment for Aadhaar Enrolment, UIDAI gives you an Acknowledgment Number then has to be written on your Aadhaar Card.

NPR Receipt/TIN Number

If you are having any National Population Register (NPR) survey slip or a Tax Identification Number (TIN) then you will have to fill this information in this field.

Name

Here you will have to fill in your Full Name including your First, Middle, and Last Name.

Gender

In this field, you will have to mention your Gender whether male, female, or transgender.

Age or Date of Birth

You will have to write your Date of Birth including Date, Month, and Year in this field

Address

Here you must write your Full Residential Address as mentioned in the document submitted by you.

Family Member Details

Write your family members' details in this field like the Aadhaar number of parents, partners, children, etc.

Applicant’s Consent

You are free to allow or deny UIDAI to share your details mentioned in the Aadhaar Form to share it with various agencies.

Bank Details

It is not a mandatory field to fill out and you have to fill in your bank details if you are willing to fill it.

Documents

If you are unable to present your document then you can use the credentials of the introducer or HUFs in this field of the Aadhaar Form.

Signature

In order to validate all the information written in the form, you will have to provide your signature or thumb impression at the end of the form.

Aadhaar Card Correction Form Contents

The Aadhaar Correction Form is the same as the Aadhaar Form. You can use this form to make corrections to your Aadhaar Card. The corrections in Aadhaar can be made online as well but if you don’t have your registered mobile number then you will need to fill out the Aadhaar Correction form and submit it at the Aadhaar Seva Kendra.

The contents of the Aadhaar Correction Form are as follows:-

• Name
• Gender
• Provide your phone number and ask him to link the number
• The applicant’s date of birth in the format DD/MM/YYYY
• Residential address
• Email ID of the applicant
• Aadhaar Number
• Parent’s name
• Mobile number
• Email ID
• Documents details for correction purposes like POI, DOB, and POA
• Signature or thumbprint of the applicant

Points to Remember Before Filling Aadhaar Form

• You must fill in all the details in the Capital letters in the Aadhaar Form.
• Make sure you mention the correct Date of Birth if you are filling the Aadhaar Correction Form because you can change your DOB only once.
• It is optional to give your mobile number in the Aadhaar Form.
• You must mention the exact details in the Address field as mentioned in the document submitted by you.
• While rectifying any Aadhaar detail through the form, make sure you don’t enter the same detail again.
• Your Aadhaar Application or Correction process will only be successful after the verification of all the details by UIDAI.
